clinic WHITE SMILE, company Luxlite Europe s.r.o. with registered office at Čakajova 25, 811 05 Bratislava, ID number: 35880651, represented by: Peter Kivader – managing director (hereinafter referred to as “WHITE SMILE clinic”)
consent to the processing of provided personal data (name, surname, telephone number, e-mail address, service data) for the purpose of recording agreed dates in the internal calendar of the WHITE SMILE clinic, providing services, creating and offering products adapted to your individual needs, at purposes of statistical evaluations and assignment of other data that are directly related to the purpose of processing.
Your personal data will be securely stored, collected, processed and archived for a period of 10 years from the date it was provided or from the last appointment with a doctor or dental hygienist at the WHITE SMILE clinic.
You have the right to delete all your personal data without undue delay, unless they are necessary for the purpose for which they were collected or processed and after the expiration of the legal reason for their eventual archiving.
The WHITE SMILE clinic guarantees that your personal data will only be accessible to employees and associates of the WHITE SMILE clinic to the extent reasonably necessary for the provision of services and the processing of personal data.
We have modern control, technical and security mechanisms, ensuring the protection of processed data against unauthorized access or transfer, against their loss or destruction and other possible misuse.
The WHITE SMILE clinic expressly undertakes that your personal data will not be provided to any other persons or third parties.
You hereby confirm that you provided personal data freely as well as that you were timely and properly informed about the processing of personal data, about the right to access personal data and the right to correct incorrect, incomplete or out-of-date personal data as stipulated in Act No. 18/2018 Coll. on personal data protection and GDPR: European Parliament and Council Regulation (EU) 2016/679 and EU Directive 2016/680.
